The Registrar will then issue you with a certificate of Burial or Cremation (Green Form), which should be handed to your funeral director. A White Certificate is issued and this can be used in any liaison with the DSS. The Registrar will also issue copies of the Registrars entry, which is required for Banks, Buildings Societies, Insurance Companies etc.
*Further information the Registrar requires:
1. The date and place of death. 2. The deceased’s date and place of birth. 3. The deceased’s full name (plus maiden name if appropriate). 4. The deceased’s occupation (or that of husband). 5. The deceased’s usual address. 6. Details of the deceased’s pension and/or allowance from public funds. 7. The date of birth of the surviving widow/widower (if married).
